The aim of this surgery is complete tumour excision with negative resection margins. To achieve this aim, in certain clinical situations, it is appropriate to resect the ano-rectum en bloc with the posterior vaginal wall. The advantage of this approach is note only to increase the chances of a negative resection margin in anterior lesions but also to excise the lymphatic channels contained in the recto-vaginal septum.
